Denison ISD Athletic Paperwork & Physicals Instructions

• A Medical History and Pre-Participation Examination Physical is required for ALL student athletes, grades 7th-12th,for the 2025-2026 school year.
• Medical History and Physical forms should be completed on the DISD pink form.
Athletic Paperwork:

ALL forms EXCEPT for the Pre-Participation Physical and Medical History will be completed and submitted using the RankOne online based software system.

• To access RankOne, go to: https://denisonisd.rankone.com and proceed to the online forms.
• Please ensure the 2025-2026 school year is selected.
• Do not use athlete nicknames on forms. Use the student’s full legal name.
• The athlete’s student ID number is required on each form.
• There are 7 forms total that must be completed.

All forms (Online, Medical History, & Pre-Participation Physical) MUST be complete and on file PRIOR TO PARTICIPATION in any practice, scrimmage or contest before, during, or after school.
